What You’ll Need

Opening your Representative Payee Account is easy. Just bring the following items into your nearest Ridgewood branch:

  • Your Photo ID
  • Your Social Security number
  • Proof of your New York State address
  • The beneficiary’s Social Security number and date of birth

You’ll also need to provide one of the following:

  • A letter of appointment from the Social Security Administration (SSA) 
  • A check made out to the representative payee
